Vital Records

The Town Clerk issues certified copies of births, deaths, marriages, and divorce certificates to qualified individuals and agencies that provide a "direct and tangible"  Section 5-C:9 Disclosure of Information From Vital Records. (state.nh.us) interest in obtaining a record.

In order to obtain certified copies of Vital Records, you must complete an application and pay a search fee. The current fee is $15.00. This fee is payable whether or not a record is found. Additional copies of the same record issued at the same time are $10.00. Cash, checks (payable to the Town of Woodstock), and credit/debit cards are accepted. 

To obtain certified copies of a birth or death certificate, the following information is needed for the application:
  1. Name at birth or death
  2. Date of birth or death
  3. Place of birth or death
  4. Mother's First & Maiden Surname
  5. Father's Name
  6. Purpose for the certificate
  7. Relationship to the person in #1
To obtain certified copies of a marriage or divorce certificate, the following information is needed for the application:
  1. Name of Person A (Groom/Husband)
  2. Name of Person B (Groom/Wife)
  3. Date of marriage or divorce decree 
  4. Place of marriage(Town) or divorce decree(County)
  5. Purpose for the certificate
Effective January 1, 2005, all individuals requesting a certified copy of a record must present positive identification, including, but not limited to, a driver’s license, passport, or other government issued picture identification, or in the absence of acceptable picture identification, shall complete a separate form. A request must contain a signature of the person requesting the record.

Vital Records
Purchase online

Records below can be purchase through any NH Town Clerk's Office.  Year prior to the dates below have to be purchase through the Town Clerk's Office where the original document was filed or obtained:
Birth Records: 1935 until present (except 1950)
Death Records: 1965 until present
Marriage Records: 1960 until present
Divorce Records: 1979 to within 6 months from the present search date
Civil Unions: 2008 and 2009
Civil Union Dissolution: 2008 to within 6 months from the present search date
 

Exempt Records: Certain older records are exempt from the access restrictions. Birth records older than 100 years, and deaths, marriages, and divorce records older than 50 years are available with unrestricted access. You can access these records through the Town Clerk where the original license was filed or obtained (please call to schedule an appointment) or through the Bureau of Vital Records Genealogy Center at 71 South Fruit Street, Concord, NH. The center is open to the public at 8:30 am to 11:45 am and 12:15 pm to 4:00 pm, Monday through Friday.
